Part 2 of the advance directive form is about life support.
Life support is not necessarily just CPR and a feeding tube. It can be as something as simple as antibiotics or dialysis. You get to choose your own definition of life support! You can cross out whatever treatments you do not want and write in whatever treatments you definitely want.
You can also vary your definition of life support depending on the conditions you may develop. For instance, if you are:
a) close to death
b) permanently unconscious
c) have an advanced progressive illness (Alzheimer's, cancer, etc.)
d) are under immense pain
It is important to be as specific as possible so that you can avoid your family members or physicians misinterpret your wishes.
